I found this on another site, so I dunno if it works or not. It's an offer from Orbit gum. You buy their gum and the enter a serial number that's on the package into the form and you'll get a free pair of flip flops. This part is true enough, but I have a code that MAY OR MAY NOT work. I signed up for them using the code myself and no problem, though I may never receive them. Who knows? Worth a shot since the form is SIMPLE.
